Passport applications are
accepted at the Tangipahoa Parish Clerk of Court's Office's in Amite and
in Hammond.
If you are getting your passport in our Hammond office you will need to arrive by 3:30 pm.

What you need to have
with you to apply for a Passport:

Certified Copy of Birth Certificate (No Birthcards)
- One - 2 inch x 2 inch photographs from a professional
studio (no photo booth). Photograph

*** 2 Parents or 2 Legal Guardians must appear and execute
the application (DSP-11) along with the applicant if applicant is
under the age of 16. (as of 02/01/2008)
Both parents of child's legal
guardian must:

- Present evidence that they are the parents
or guardian and
- Show valid personal identification and
- Sign and take oath before an authorized
passport acceptance agent.
